I acknowledge that while safety is a priority, accidents can happen, and I voluntarily assume all risks associated with my child’s participation.
  2. Release of Liability: I hereby release [Your Name/Club Name], its instructors, and [Community Hall Name] from any and all liability, claims, or causes of action arising out of any property damage, personal injury, or illness that may occur during or as a result of participation in the club.
  3. Supervision & Pickup: I understand that [Club Name] is responsible for my child only during the scheduled class time. I agree to drop off and pick up my child promptly.
  4. Art Materials: I acknowledge that some art materials may stain clothing or skin. [Club Name] is not responsible for damage to personal belongings or clothing.
  5. Medical Consent: In the event of a medical emergency, I authorize [Club Name] staff to seek professional medical treatment for my child if the emergency contacts provided cannot be reached.
